I will now explain the principles of punctal occlusion surgery in just three minutes.

The direction of the arrow corresponds to the path that tears take (from the eye to the nose).

In this pathway, the entry point from the eye to the nose is called the "tear duct" or "punctum."

Punctal occlusion surgery involves blocking the punctum with a harmless substance to prevent tears from further drainage, thus retaining moisture in the eyes.

Looking at the image below may help you understand better.

The purple bar you see is called a plug.



By inserting the plug snugly into the punctum, we can block the tear duct with a harmless substance, and there is minimal pain involved.

(As a result, when tears cannot flow down to the nose and are retained in the eyes, the area around the eyes becomes moist, right? ^^)

The plug is made of a collagen component that dissolves over time, naturally melting within the eye.

Depending on the material used, it can be adjusted for a period of 1 week to 6 months or even permanently. However, for safety reasons, we prefer to use a product that lasts for at least 6 months or longer.

One of the advantages is that there are no major side effects due to the dissolvable nature of the plug, and that is why so many people have sought this treatment.



However, there is one thing to be cautious about when undergoing this procedure.

After receiving punctal occlusion surgery, if you forcefully blow your nose or vigorously rub your eyes, 

the plug may come out. Additionally, if the doctor does not insert the plug to the appropriate depth and leaves it slightly protruding, it may cause some discomfort and irritation to the eye.

While you can control the act of blowing your nose or rubbing your eyes, 

the depth of insertion is within the doctor's expertise. Therefore, I recommend visiting a clinic or hospital that has sufficient experience in performing this procedure.^^

(In our case, the entire procedure for both eyes usually takes about 1 to 2 minutes, hehe.)



Punctal occlusion surgery cannot treat all cases of dry eyes.


In some cases, the underlying cause of dry eyes is not simply a lack of tear production, but rather a dysfunction in a different part called the meibomian glands.

The meibomian glands are located in the upper eyelid when it is turned inside out. They produce an oily substance that helps coat the eyes and prevent tears from evaporating quickly.



When examining the infrared image of the right eye, you can see that a portion of the meibomian gland is blocked due to debris.

If this area becomes blocked, it impairs the function of producing oil, which is a component of tears. As a result, the area around the eyes quickly becomes dry.

For such patients, it is more important to restore the normal function of the meibomian gland to facilitate the production of clear oil, rather than using punctal occlusion to trap tears.
